Для анализа и оценки данных литературных источников использовались материалы ресурсов E-LIBRARY, PubMed, Google Scholar и Elsevier Science Direct. Изучены научные работы, посвящённые вопросам воздействия ионизирующего излучения на состояние зубов после лучевого лечения злокачественных новообразований челюстно-лицевой области. Представленные обобщённые результаты клинических и экспериментальных исследований показывают специфическое влияние дистанционной лучевой терапии ЗНО ЧЛО на особенности проявления патологии зубов, характеризующиеся интенсивным разрушением зубов в ближайшие и отдалённые сроки после лечения. Дистанционная лучевая терапия, являющаяся основным методом лечения ЗНО ЧЛО, способствует возникновению морфологических изменений тканей зубов. Изменения в твёрдых тканях и пульпе зуба имеют непосредственную взаимосвязь с проведением лучевой терапии при ЗНО ЧЛО. На сегодняшний день среди специалистов отсутствует единое мнение о патогенетических механизмах лучевой деструкции тканей зубов и пульпы. Некоторые авторы характеризуют данные постлучевые изменения органов и тканей полости рта как острый множественный кариес зубов, а некоторые исследователи относят данный патологический процесс к некариозным поражениям, которые развиваются после прорезывания зубов. У больных с ЗНО выявляется различный характер поражения твёрдых тканей зубов в зависимости от места приложения лучевого воздействия. После лучевого лечения в области головы и шеи наблюдается некроз эмали и дентина, осложняющийся кариесом и острым быстротекущим кариесом, а после лучевого лечения в области грудной клетки и тазовых органов бывают осложнения в виде острого и острейшего кариеса, а в отдельных клинических случаях – пришеечный некроз эмали зубов. В этой связи проблема изучения патогенеза лучевого повреждения зубов является актуальной для повышения эффективности реабилитации онкологических больных и формирования тактики выполнения комплексной медико-социальной реабилитации пациентов. Проблема состояния зубов после дистанционной ɣ-терапии ЗНО ЧЛО остаётся неизученной в полной мере и требует проведения дальнейших исследований с целью определения тактики диспансерного наблюдения и особенностей применяемых лечебных, профилактических и реабилитационных стоматологических мероприятий.</p></abstract><trans-abstract xml:lang="en"><p>This systematic review presents data from literature sources on the development of pathological processes in teeth following radiotherapy for malignant neoplasms (MN) of the maxillofacial region (MFR). Materials from E-LIBRARY, PubMed, Google Scholar, and Elsevier Science Direct were used for analysis and evaluation. The sample consists of scientific papers devoted to the effects of ionizing radiation on the condition of teeth after radiation treatment for malignant neoplasms of the maxillofacial region. The generalized results of clinical and experimental studies presented in this review demonstrate the specific influence of remote radiotherapy for MN of the MFR on the characteristics of dental pathology manifestation, both in the short and long term after treatment completion, characterized by intensive tooth decay. Remote radiotherapy, being the main method of treatment for MN of the MFR, contributes to the occurrence of morphological changes in dental tissues. Changes in hard tissues and dental pulp are directly related to radiotherapy for MN of the MFR. However, there is currently no consensus among specialists regarding the pathogenetic mechanisms of radiation-induced destruction of dental tissues and pulp. Some authors characterize these post-radiation changes in the oral cavity as acute multiple caries, while others attribute this pathological process to non-carious lesions that develop after tooth eruption. Furthermore, patients with MN exhibit different patterns of hard dental tissue damage depending on the location of radiation exposure. After radiation treatment in the head and neck region, enamel and dentin necrosis complicated by caries and acute rapidly progressing caries are observed, while after radiation treatment in the chest and pelvic organs, acute and hyperacute caries are observed, and in some clinical cases, cervical enamel necrosis. In this regard, the problem of studying the pathogenesis of radiation-induced tooth damage is relevant for improving the effectiveness of rehabilitation of cancer patients and developing tactics for complex medical and social rehabilitation.
